Integrating care is promoted by the World Health Organization as a way to decrease care fragmentation and to fortify people-centered systems. This poster demonstrates how the Relational Coordination model was utilized to integrate social work and audiology to improve mental health access for people with haring loss.
Learning Objectives:
Upon completion, the participant will be able to describe the Relational Coordination Model.
Upon completion, the participant will be able to identify why integrated care is needed for individuals with hearing loss.
Upon completion, the participant will be able to explain how to integrate care through the Relational Coordination Model to social work stakeholders such as professional social workers, researchers, instructors, and to social work students.
